Upham’s Elder Service Plan supports families and caregivers so that their older loved ones can continue to live at home instead of in a nursing home.

We provide caregivers:

  • Individualized consultation on care plans
  • A Care Team that works closely with participants and their caregivers to meet a participant’s health and personal needs
  • 24-hour telephone access for emergencies and unexpected situations
  • Respite services
