You can start the process today by calling us at 617-740-8007 or filling out the enrollment form, or emailing us with your contact information to schedule your home visit.

Enrollment and dis-enrollment to a PACE program are voluntary. Status changes for PACE participants are always activated on the first of the month.

In order to be eligible, you must meet the following requirements:

  • 55 years of age or older

  • Reside in our service area

  • Certification by the state screening agent that the applicant meets nursing facility clinical criteria.

  • Must have the ability to safely live at home with UESP support services.
